Griffiths discharged patient from psychicatric hospital who was in a severe psycosis without informing a family member. Patient had a serious motor accident two days later and could have lost her life or somebody els could have lost their life. Patient was taken back to psychicatric hospital imediatly after accident and Griffiths contacted mother telephonicaly and admited to mother that patient was in a severe psycosis and should be certified.
