On 9/8/07, I forwarded a Rebate Form & copies of bills for windows purchased at Home Depot and installed in my home at the above address with a request for Michael Dunn to fill in and sign the bottom half of the enclosed form and to then forward it to GasNetworks Energy Star Windows in a self-addressed envelope that I enclosed for him to mail to them.

As of today, I have not received a rebate check from GasNetworks Energy Star and I am also sending them an email regarding this matter.

Please check with Michael Dunn to see if he did in fact fill in the rebate form and then mailed it to GasNetworks in the envelope addressed to them that I had enclosed for his convenience.
