I called for an appt to have one crown recemented, one dental implant and dental cleaning. After exam, the office mgr presented me with a dental treatment form and asked me to initial te spaces/blocks and sign the form. most individuals do not know their by numbers. I initialed and signed the form, acknowledging that I had seen the form. I was not advised that they planned to perform the dental work that they had specified. They proceeded to remove 2 Gold crowns, and processed another tooth that been filled by my family dentist 3 months prior. I told him no, he did not listen and put a crown on that tooth. I realized, I was being scammed and they planned to charge me $14, 000 for all the work they had listed. I said no and never returned. They now have charged my credit card $1, 200 for the crown, I had denied, without my acknowledgement, approval or signature. The credit card has refused to accept my complaint. I am still disputing that unauthorized charge and the crown he made without my approval and refused to listen to me. What do I do now? I have already paid him $4, 000 for services I did not want or need. I refuse to pay for the crown was not necessary and that I did not. He uses his Treatment Form ass approval to scam his patients.
