I, Md Anwar Ali S/o Md Saukat Ali age about 38 years, Occupation- Advocate, P.S.- Sundargarh Town, Dist- Sundargarh, beg to state as follows :-

1. That, I am one of the Indane Gas Consumer bearing Card no – B-10549, resided at – Bijaya Talkies Road, Po- Sundargarh, Dist- Sundargarh.

2. That, since June .2010, I have received one Gas Cylinder refill from the said agency and several time I have made request before the dealer for supply of Gas Cylinder but surprise that others customer have got Cylinder refill twice or thrice in a month but the Gas supplier did not give me a single Gas Cylinder in time. I have also submitted the Gas Card before the Gas agency since 18.08.2010 but the gas supplier would not like to supply gas.

3. That, the Gas agency did not issue any receipt against the Gas card deposited before the Gas Agency .

4. That, the Gas agency did not maintained any record against the booking of cooking gas is suspicious.

5. That, the Gas agency should issue receipt against Gas Card deposited or issue a serial number of booking made by the consumer after enter the name and card number in a proper manner .


6. That, the Gas agency should display the stock position, rate and supply of refill Gas cylinder per day for the consumer.
