My credit card company changed their format for statements and it took me the third statement to realize there was a charge that I didn't recognize. Each month I was being charged $11.97 from CMG Free Shipping.com for something that I never authorized. I've had to close out my credit card and file forms to dispute the charges. I have no idea how they got my card number!!
