I had purchased the Policy bearing No - 4034/01971218/FFH/W-048806 from your Co.
against which I had issued cheque bearing no - 495055 dated 23.10.07 drawn on SBI, Mumbai
for Rs. 14,061.00 which has been already debited to my account on 31.10.07.

I regrett to bring to your notice that till now I have neither recd the Insurance Certificate nor the Cashless Card . I fail fail to undertand such irregularity from your end.

Plz let me know whats happening throught the returning mail & also send me the above documents & Cashless Card immediately

If, its not possible for your company kindly refund me my money immediately.
