I did attempt to claim a gift card I had won, but did not ask for a membership. To make matters worse, I was trying to appeal medicaid, finding out my past year of treatments (when I was out of work) was not working, and facing surgery in October, and going thru counseling weekly. My mail has been scattered, and I just recently learned my credit card has 4 charges. I have only recieved two, unless my "trial" bottle was a third. I want these refunded. I want to close this disaster. I need the return authorization number to return the shipments (2) that I have.
