I was solicited by email/phone for group health insurance. The phone representative told me there was a one-time association fee of $450 plus the monthly premium/charge of $434 per month. Gave them my bank account info to withdraw this amount. Got a bank statement about 2 weeks later showing there had been five (5) withdrawals totaling more than $1200 - all this more than 2 weeks before my coverage was to begin.

I immediately closed the bank account and followed all procedures for cancellation, also demanding a full refund of all withdrawals from my bank account. Still have not received a refund of any withdrawals.
