I and my wife are holder of a Savings Bank Account in Punjab National Bank GD 24, Sector III, Salt Lake, Kolkata – 91. Our savings Account No. is 0139542. As per my records, an amount of Rs. 15, 000 /- was last withdrawn on 09.09.2008. Recently, I had deposited a cheque ( No. 795197 dated 01.02.2011) for Rs. 1, 000/- for clearing from your bank at S.B.I., Bangur Avenue, Kolkata - 55.
