I completed a survey for the company and gave my phone number of 601-562-1294 and zip code of 36571. I was then told to complete the information on www.survey claim.com. I did so but they had my last name spelled incorrectly as Bigha. I edited information as instructed and changed last name to Bingham plus added a home telephone of 251-675-3113. I agreed and completed my confirmation and got a message that my information was inconsistent.
