Do not apply for the "free" trial from Ivory Brites. They automatically sign you up for online Axis Wellness Portal whether you check the box or not. When you call and complain about the $9.99 a month charge for this unwanted service you are required to email your complaint. I DID NOT check the box for this service and they claim that I did.
