On May 15, 2009, I authorized a one time charge of 4.99 for a trial offer from My Whitening/Max White/Axis Wellness Portal (they use all three names in this scam). In addition to the money provided for the trial offer, My Whitening/Max White/Axis Wellness Portal has charged my account an additional 78.93.
