I too, ATTEMPTED to request a FREE sample of the tooth whitening product and have had my credit card charged for services I did not request. During the process of filling out the on-line FREE sample request form, I noticed a button to activate the request and two trial memberships to other clubs. I saw no way to enter the FREE sample request form and not enter the memberships so, I pressed the Esc button instead. This should have discontinued my enrollment completly.
