Back in October of 2007 I had AT&T/SBC install DSL internet service. Part of the package was a Motorola PCMCIA adapter that I didn't need but was required to purchase with the understanding that it would be free via a rebate. When the adapter arrived, the 'rebate activation sticker' was missing and where it was supposed to be was a statement that a rebate form would be mailed separately. The rebate form never came.
