On June 28, 2005, we sent in our Reservation Request Form #001045 to the office of Premium Travel. On August 16, 2008 my husband call Gina to follow up and stated that Premium Travel had no record of receiving our Reservation Request Form. My husband faxed over a copy of request form and the Money Order receipt, But did not get a promised call back.
