Policy # 207, 064, 239: On March 15. 2011, I fell off a tread mill at Life Quest Exercise Gym, and fractured my pelvis. I could not walk, and Dr. prescribed a wheel chair, which I was in for four weeks, enduring much pain. On 3/20/11, we gave Mr. Leslie S Hollo, Insurance agent, a completed claim form, detailing the accident.
