Sophia Trzyna was a resident at The Clare from February 20, 2011, to February 28, 2011, for short term rehabilitation, nursing care, and supervision following a surgery. The Clare caused the dehydration, malnutrition, and an abdominal infection of Sophia Trzyna. The dehydration and malnutrition resulted in renal failure. The abdominal infection was caused by poor ostomy care. The improper and inadequate care of Sophia resulted in hospitalization at Northwestern Hospital and is related to her subsequent death. read full review »
