I was at sam's Wholesale Club on the 06/24/11 at 16:57 I went to the Service Desk to show my tax Exempt card the lady Margaret told me I had to do another form the streanlined sales Agreement I told her I did that this is how I got my card.Threw the North Carolina Department of Revenue.
