I can not get the FMLA paperwork I need completely filled out by the doctor. The first time I went in, the office staff went out of their way to help me. But, the form was not filled out properly. So I went back, and this time I was told I had to make an appointment. My mother, who is the patient, had to see the doctor so she made an appointment and took the paperwork.
