My name is Tom Anderson and I am formally filing a complaint about the services I recieved when I visited the emergency room at Providence Saint Josephs Medical Center (501 S Buena Vista St, Burbank, CA) on Monday, November 10th, and against all the contractors involved in my "treatment" that day/evening.
