Form-A-Corp processed my credit card to form an LLC in January 09. They filed for an EIN number with the IRS (which can be done for free) and never completed the process. They do not answer the phone or emails. Because forming a corporation can take some time, I lost the time frame to get a refund from American Express. PayPal attempted but was unable to get in touch with this company either. I sent a complaint to the BBB which also was unsuccssful. This site continues to advertise and even offered a summer special.
