My dispute is $77.00 for Optometrist Services provided on 2/3/2011 from Richard G. Alexander, Optometrist, (Tax Id: 02-0532850) Schaumburg, Illinois. He gave me an eye examination and put (2) RX Lens with standard antiglare and transitions lens into a pair of eyeglass frames I provided. The total cost I paid for the examination and services was $390.00. As a member of Humana Insurance, the vision coverage is a discount program with Eye Med Vision Care. I experienced a similar situation where I made an early payment that did not post to the correct billing cycle. Despite numerous phone calls to insure that the payment was applied/posted correctly. CS Reps. did not record any phone calls or request. The two payments were posted to the same billing cycle - SURPLUS.
