I called my local company Qwest regarding an odd charge on my last page. I was fully unaware how it had gotten there. Qwest forwarded me to ESBI. When I called ESBI I was told that my birthdate suggested that I approved my intent to add their service. It has been quite some time so obviously my fault for not catching it sooner. But how they got me is beyond me.
