A few months back I was treated by a physician and had several blood tests performed. At the time of that visit, he had explained there would be a $149 out-of-pocket expense for some testing, and I was given materials needed for submitting 4 timed saliva and urine samples - along with paperwork showing prepayment of $149. Between my husband being out of work for over two years and myself now out on unpaid medical leave, I chose not to pursue that part of the testing.
