I too received this $14.95 montly charge after making a purchase on ProFlowers. After finally getting through to EZ Saver Rewards, they explained that I must have clicked a link offering me %15 off my next purchase coupon. Once on that page, I filled out my e-mail, last name, and zip code. My mistake was that I did not read the terms and conditions, which I guess tells you that you are signing up for a monthly service with a free 30 day trial.
