I have been a patient at Johns Hopkins hospital and its outpatient clinics for the last ten years. For the last year I have tried four times to get a copy of my medical records sent to both me and the Department of Veterans Affairs (VA). All three times I stopped by their medical records office and filled out the request paperwork. The first two times I called a week later to check to see if the records had been sent, and I was notified that yes, they had been faxed or mailed the next day.
