Date of issue: January 4, 2009 Time of issue: Between 11:30am-2:00pm My wife and I are currently on state insurance and unemployed. On the date above my wife was scheduled to go to her pain management appointment. Going there was fine but, on the return trip to the pharmacy the driver started to make a left hand turn when he was supposed to make a right hand turn into Walgreens parking lot.
