A few months ago I received a call from an overly aggresive person saying they were representing Bank of America. They were offering a credit protection plan for 12.99 a month. I told them no, but agreed to have the materials sent to me for approval. My answer "yes" to the sending of materials was enough to activate the program and the monthly charges began.
