After going to the drugstore and finding that I had used up all my benefit for Medicare D and went to pick up my additional 4 pills at the drugstore - to find that the cost was s$129.00, They said the Avelox was not on the formulary. I called to ask what po (by mouth) antibiotics were on the formulary. I had just finished Levequin which was on the formulary but md wanted to give me a different one.
