I received one of these letters in the mail today from UBI Payment Services. It claims that I have won $5000.00 dollars payable in the form of a check. All that is needed is to sign the release form and mail it back. The problem is that I never entered any such contest and the return address is a P.O. box number: P.O. Box 4805 in Robina Town Center in Australia. This is a common mail fraud scam, your Identity is stolen and since the return address is a P.O. box number that is outside of the USA.
