March 2008 <br /> My first visit to the West Coast Dental office located at 5162 Whittier Blvd Los Angeles, CA 90022 was on a Friday in the middle of March 2008. Upon my arrival I checked in with the Receptionist Sam. Sam requested my medical card and in turn provided me with the necessary forms for me to fill out. I completed and returned the forms and given my medical card back. Sam then requested my driver’s license. I turned over my license.
