On January 14, 2009, I visited Dr. Carl F. Asseff for a problem with my eyes. Dr. Asseff treated me for an eye infection related withthe contact lenses.I went there for a several visits, on acerage every three to four weeks. Dr. Asseff collected his fees to the last decimal point from my helth insurance and myself. The last visit was on April 25, 2009. Dr. Asseff gave a prescribtion for new lenses.
