I had GEICO for several years, when my daughter turned 18, in September of 2008 we had her get her own insurance policy. In December of 2008 she let her policy lapse. January 1 of 2009, I sat her down and we went through progressive where she got a new policy for herself. Apparently GEICO readded her to MY policy on January 28, 2009 as her policy had lapsed. They never notified me, couldn't tell me what the increase was in my policy (it was renewal). They never asked me if she had coverage.
